Background: Typhoid fever remains a highly contagious and multi-systemic infection caused by salmonellae typhi, like other gastro-enteric infection, it poses higher risk of morbidity mortality during pregnancy as result the physiological changes associated with well concerns antibiotic safety in pregnancy. The occurrence typhoid perforation may be easily confused for causes acute abdomen pregnancy.
 We report presentation, management outcome 25 year old postpartum woman who presented intestinal resource limited environment. She had an emergency exploratory laparotomy was managed post operatively ICU but clinically deteriorated subsequently died on third post-operative day secondary to multiple organ failure from severe sepsis.
 is rare high index suspicion, early intervention, would reduce incidence this disease condition.
